Eight things, in four pairs, each one active and passive
Umm Salamah says he never once left the house without it.
اللَّهُمَّ إِنِّي أَعُوذُ بِكَ أَنْ أَضِلَّ أَوْ أُضَلَّ أَوْ أَزِلَّ أَوْ أُزَلَّ أَوْ أَظْلِمَ أَوْ أُظْلَمَ أَوْ أَجْهَلَ أَوْ يُجْهَلَ عَلَىَّ
Allāhumma innī aʿūdhu bika an aḍilla aw uḍall, aw azilla aw uzall, aw aẓlima aw uẓlam, aw ajhala aw yujhala ʿalayy
“O Allah, I seek refuge in You from going astray or being led astray, from slipping or being made to slip, from wronging or being wronged, from behaving ignorantly or having ignorance shown to me.”
Where this comes from
She says he never came out of her house without raising his eyes to the sky and saying it. Each pair is the same fault twice, once as the one who does it and once as the one it is done to, and both are asked about equally.

Narrated Umm Salamah, Ummul Mu'minin: The Messenger of Allah (ﷺ) never went out of my house without raising his eye to the sky and saying: "O Allah! I seek refuge in Thee lest I stray or be led astray, or slip or made to slip, or cause injustice, or suffer injustice, or do wrong, or have wrong done to me
